As a part of the contract you signed with the University of Wisconsin-___________ for services, an insurance certificate was required. To date we have not received this certificate. Please refer to the contract for the requirements. As the service you are providing is valuable to the University of Wisconsin-_______________, please forward a current certificate of insurance to my attention. We need to receive this certificate via certified mail within 15 days as failure to send the certificate via certified mail within 15 days, to my attention, will result in the cancellation of this contract.
